The Site Medical Director has overall responsibility for providing and ensuring quality medical care to patients at medical site including the direct management and supervision of site medical clinicians.nbsp; Recognizing the need to balance the needs of the Residents and the patients in need of care, as an Open Door leader, seeks to promote Open Door#39;s patient-centered values and style="font-family:Calibri,sans-serif"span style="color:#44546a"Identify and outreach potential candidates for comprehensive sexual health education (HIV/STIs/HCV) and PrEP/PEP navigation and support program (individuals at high risk for HIV infection) by using Open Door data reports, collaboration with medical providers and outreach activities/span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Provide education to clients on prevention topics, including HIV/STI/HCV/PrEP/PEP, consistent and correct condom use, safer sex practices, harm reduction, regular STI and HIV testing, and the importance of mental health and substance use issues in relation to HIV risk/span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Refer individuals for HIV/STI/HCV testing and collaborate with medical providers to coordinate immediate follow-up after reactive/positive results including navigation to medical care (including other testing) and assistance with partner notification; for patients diagnosed with HIV, conduct a warm handoff to ongoing case management or provide ongoing case management services/span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Coordinate clinical assessments and provide education/support for individuals interested in PrEP; for those who choose to engage in PrEP care, provide education on self-management, the medication regimen and managing potential side effects, treatment adherence support, schedule follow-up visits, send reminders and follow-up on no-shows, and assist with affording PrEP meds and visits by linking to insurance enrollment or assisting with PrEP-AP and pharmaceutical assistance program paperwork/span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Coordinate care for patients in need of PEP services and discuss PrEP for future protection; navigate to PrEP care as patients are interested/span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Conduct Social Determinants of Health (SDOH) assessments to identify barriers to care/treatment and provide navigation and linkage to needed supports and services with an emphasis on physical, sexual, mental, and behavioral healthcare/psychosocial counseling /span/span/li lispan style="font-family:Calibri,sans-serif"span style="color:#44546a"Provide assistance with appointment scheduling, accompaniment, and appointment support; facilitates, coordinates and documents participant-centered care for participants across Open Door teams and departments to better address multiple and co-occurring concerns/span/span/li
