Supports and carries out/ppthe Home Care Program’s policies and procedures./ppbr//ppstrongPay Rate is $33.77 per hour/strong/ppbr//ppstrongSecurity Clearance/strong/ppLevel 1 V,A,C,R/ppRequirements/ppstrongPosition Responsibilities and Specific Duties:/strong/pp• Provides supervision and evaluation of Home Care workers according to/pprequirements for contracting agencies and DOH regulations./pp• Provides on the job training when needed and provides in home joint orientation to/ppnew personnel assigned to a case./pp• Maintains accurate patient files./pp• Maintains up to date employee health records and personnel files./pp• Performs initial admission assessment of new referrals, and opens the case, if deemed/ppappropriate for home care to be provided./pp• Completes mobile intake./pp• Works in cooperation with the scheduler to assign appropriate staff to the patient/ppreceiving home care services./pp• Reviews all patient care plans with home care workers to assure appropriateness and/pptheir ability to provide the services required. When appropriate, provides case/ppmanagement and obtains physicians orders to develop a plan of care to meet the needs/ppof the patient. Review this plan every 6 months or sooner if necessary./pp• Reviews all complaints that are filed and provides follow up for resolution to/pppatient’s satisfaction./pp• Documents and monitors all accident/incident reports, provides further investigation./pp• Facilitates quarterly QI meetings./pp• Maintains liaison with caseworkers, patients, families, and staff./pp• Maintain aid certifications in NYS DOH database./pp• Obtain and maintain aid certifications for NYS core curriculum./pp• Provides on-call coverage./pp• Promotes agency mission./pp• Job performance incorporates integrated service delivery model while performing/ppself-sufficiency./pp• Participates in committee structures, planning, policy development, and volunteerism./pp• Provides and participates in cross training as needed within the division and COI./pp• Coordinates certification training for personal care aides on an as needed basis./pp• Administers equivalency testing, including written skills and demonstration of skills./pp• Maintains current knowledge of and assures regulatory and procedural compliance./pp• Assists with recruitment and hiring of new aides./pp• Interviews new applicants and recommends appropriate candidates./pp• Class preparation, researching current changes in regulations./pp• Administers the NYS Home Care Core curriculum as required by DOH./pp• Includes 40 hours of classroom training, skills training, hands on experience,/ppDDHCW training (A Better Answer), and administers final exam./pp• Provides on the job training./pp• Schedules in services countywide and coordinates their presentation./pp• Works in cooperation with the Health and Family Services Director to develop, plan,/ppand implement new training programs./pp• Works with internal and external partners to provide Health Care Consulting services./pp• Works with team on PR and recruitment strategies; community relations./pp• Develops and keeps procedures and protocols current; implements designated/ppmanagement systems./pp• Assists with providing and developing training and orientation for service area staff./pp• Promotes agency wide integrated service delivery strategies./pp• Participates in designated Coalitions./pp• Fosters positive relationships with peers, funders, licensors, staff, subcontractors and/ppcommunity partners./pp• Monitors identified service area budgets; assists Supervisor to develop new or/pprenewed budgets./pp• Facilitates and participates in meetings as designated./pp• Fosters ongoing, integrated, comprehensive service delivery model with other COI/ppservices./pp• Coordinates designated functions of the MOU and MOA process./pp• Facilitates standardization of systems and processes within service area and division./pp• Utilizes CAPSYS, data, and other tracking and measurement tools to monitor staff/ppperformance and service area outcomes; drive improvement and response to need in/ppservice area./pp• Promotes positive agency, division, and service area image./pp• Other duties as assigned by supervisor./ppbr//ppstrongKey Working Relationships:/strong/ppA. Titles supervised: PCA I, PCA II, HHA’s, Home Care Clerk Scheduler/ppbr//ppstrongOrganizational Responsibilities:/strong/pp• Adheres to all policies and procedures./pp• Attends appropriate in-services/trainings./pp• Attends all required meetings./pp• Data collection, analysis amp; reporting as required./pp• Participation in all management systems and functions internally and externally/pp• Participates in organizational committee structures as appropriate./ppbr//ppstrongPhysical/strong/pp• Ability to lift up to and including 25-50 pounds of physical effort./ppbr//ppstrongKnowledge, Skills Required:/strong/pp• Bachelor's degree in nursing required./pp• Current NYS license for Registered Professional Nurse – DOH fingerprinting already/ppcompleted for licensure./pp• Two years’ experience working with patients; three years preferred./pp• Two years’ supervisory experience required; three years preferred./pp• Ability to provide leadership and supervision./pp• Demonstrate writing and organizational skills./pp• Ability to develop a training plan based on the Home Care Core Curriculum./pp• Ability to problem solve; advocate and provide outcomes for the patients./pp• Customer service skills./pp• Basic computer skills./ppbr//ppstrongSpecial Requirements, if any:/strong/pp• Must be able to provide consistent even tempered customer service at all times./pp• Valid Driver’s license/pp• Reliable transportation/pp• Ability to travel throughout the county./pp• Works compassionately with a diverse population./pp• Experience navigating community programs./p
