In this role, you will primarily be creating and managing a plan of care for individual patients. You will report to a Registered Nurse (RN). You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient#39;s best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. lt;/divgt;lt;divgt;lt;brgt;lt;/divgt;lt;divgt;lt;stronggt;Responsibilities lt;/stronggt;lt;/divgt;lt;ulgt; lt;ligt;Provide necessary medical care, monitoring of care, follow through of care, medication education and other related areas.lt;/ligt; lt;ligt;Encourage the normalization and independence of individuals served.lt;/ligt; lt;ligt;Provide training and assistance to individuals served and staff on medial education and other related areas.lt;/ligt; lt;ligt;Act as a liaison to outside medical providers to ensure coordination of care for the individuals served.lt;/ligt; lt;ligt;Assist in the administration of medications and required documentation for the individuals served. lt;/ligt; lt;ligt;Maintain accurate written records in connection with medical, dental, and other related matters for the individuals served.lt;/ligt; lt;ligt;Intervene when needed in emergencies with regard to the individuals served.lt;/ligt; lt;ligt;Attend interdisciplinary team (IDT) meetingslt;/ligt; lt;ligt;Experience working with individuals with developmental disabilities preferredlt;/ligt; lt;/ulgt;lt;divgt;lt;brgt;lt;/divgt;lt;divgt;lt;stronggt;Qualificationslt;/stronggt;lt;/divgt;lt;ulgt; lt;ligt;Currently Licensed Practical Nurse (LPN) in NJ.lt;/ligt; lt;ligt;Previous experience as a LPN is preferredlt;/ligt; lt;ligt;Excellent interpersonal and client care skillslt;/ligt; lt;ligt;Valid driver’s license lt;/ligt; lt;ligt;First aid/CPR certification required lt;/ligt; lt;ligt;Ability to build rapport with patients.lt;/ligt; lt;/ulgt; lt;/divgt;
