We are professional, agile, fun and our goal is to Grow our amazing group of clinicians and continue to provide excellent care./ppOur work environment includes:/pulliModern Office Setting/liliEfficient Team Work/liliLively Atmosphere/liliGrowth Opportunities/liliKnowledgeable and Dedicated Staff/liliFrequent Employee Recognitions /li/ulpResponsibilities:/pulli Assess potential Hospice or Home Health patients in a timely manner/liliEducate patient, family, and caregivers on Hospice and Home Health services/liliConduct patient assessments to determine their needs and develop individualized care plans/liliInclude the patient and caregiver in the development of the care plan/liliCollaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans/liliProvide direction to LPN to ensure patients needs are met/liliUtilize knowledge of physiology and medical conditions to provide appropriate care and interventions/liliPerform educational visits to potential patients, providers, or facilities as requested/liliParticipate in IDT and assist in Home Health to Hospice conversions/liliConduct utilization management activities to ensure appropriate use of healthcare resources/liliProvide education and support to patients and their families regarding their healthcare options and treatment plans/liliCoordinate delivery of needing supplies, equipments, and medications in a timely manner /liliProvide education to community partners on hospice and home health and assist in business development/liliPerform Case Management responsibilities including but not limited to updating plan of cares, performing recertifications, and supervisory visits./li/ulpSkills:/pulliIn-depth knowledge of physiology and medical conditions, allowing for accurate assessment and intervention for both Hospice and Home Health /liliProficient in conducting patient assessments and utilizing diagnostic evaluations to inform care plans/liliExcellent patient service skills, with the ability to provide compassionate and comprehensive care/liliExperience in hospice and good understanding of qualifications for hospice./li/ulpbr //ppThis is an exciting opportunity for a Registered Nurse who is passionate about providing high-quality care and educating the community on the benefits of Hospice and Home Health.
