Enable consumers to obtain or maintain an independent living situation that allows them the least restrictive environment by helping them remain in the region and in their own homes./ppUphold the organizationrsquo;s vision, mission, and corporate values. Demonstrate understanding of and compliance with organizationrsquo;s policies, procedures, code of conduct and work rules./ppEssential Functions:/ppbull; Provide outreach services as a primary worker with clients, parents and families, becoming completely familiar with consumerrsquo;s Individualized Service Plan (ISP) and implementing assigned duties from that plan./ppbull; Promote learning about social skills, boundaries, living skills and hygiene, assisting the development and promotion of community awareness programs./ppbull; Provide personal care as noted in the consumerrsquo;s ISP, including bathing, dressing, transferring, etc. Provide chore service as noted in the consumerrsquo;s ISP, including light housekeeping tasks, laundry, shopping, etc./ppbull; Maintain daily reporting through case notes and Medicaid documentation./ppbull; Participate in treatment team meetings when necessary and assist in planning and implementation of recreational, educational, and vocational activities./ppbull; Motivate and encourage consumers to complete required treatment activities by developing positive empathetic relationships with consumers./ppbull; Provide crisis intervention as needed./ppbull; Coordinate with community professionals as required, including NSHC personnel, and personnel from other agencies, etc./ppPersonal Care Attendant; 02-14-2020; Rev. 8/5/2021 Page 2/ppbull; Attend pertinent training and departmental meetings as requested.Participate in activities that promote professional growth and self-development./ppbull; Perform other duties as assigned./ppPersonal Traits:/ppAll Norton Sound Health Corporation employees are expected to consistently demonstrate the organizationrsquo;s values: integrity, cultural sensitivity and respect for traditional values, continual learning and improving, compassion, teamwork and pride.
