This role ensures that each personrsquo;s needs and goals are met through a person-centered approach./p hr / h2strongMajor Responsibilities/strong/h2 h3strong1. Service Coordination/strong/h3 ul liFacilitate and document the person-centered planning process, including development, implementation, and management of the Individual Service Agreement (ISA)./li liComplete all required documentation accurately and on time./li liAssist individuals in identifying strengths, needs, and goals; maintain regular interaction with assigned clients./li liReview ISA expectations, objectives, and data collection systems to ensure consistent implementation and compliance./li liCoordinate services with other providers (e.g., physicians, specialists, therapists, psychologists) as needed./li liConduct regular face-to-face visits in the individualrsquo;s home and community, including at least one home visit per month./li liEnsure adherence to all state regulations./li /ul hr / h3strong2.
