Join a team that’s breaking down boundaries to deliver proactive, preventative screenings at no cost to the patient./ppstrongWhat You Will Do/strongbr You’ll providestrong /strongannual in-home assessments (IHA) for members of Medicare, Medicaid, and Affordable Care Act plans. These 90-minute visits identify care gaps, health risks, and social determinants of health to help PCPs and health plans promote better outcomes. You will collect patient histories, document patient concerns, and provide health education. You are not responsible for lab tests, blood work, or treatment. You will be asked to:/pulliPerform high quality assessments to meet patient needs and health plan expectations./liliDocument and submit chart notes within 24 hours of the visit./liliAddress charting queries within 48 hours of receipt./liliAttend periodic training sessions and meetings pertinent to the position./li/ulpstrongWhy Our Patients Need You:/strongbr Our patients need someone who’ll go the distance for them, literally and figuratively. Many of our patients have physical limitations and social barriers that restrict their access to quality care. It’s a vicious cycle that leads to frequent hospitalizations and higher healthcare costs. The solution starts with accurate, complete documentation!br br In-home assessments help identify the social determinants of health that you can’t see from a doctor’s office. They also give patients the time they need for meaningful conversations, ensuring their questions are thoroughly addressed./pstrongQualifications:/strongpstrongOur Ideal Candidate /strong/pulliDemonstrates empathy in every patient interaction./liliCan make critical decisions while following Emcara Health policies and procedures./liliHas an advanced understanding of risk adjustment models (or is curious to learn)./liliIs proficient with electronic medical records (EMR)./liliIs comfortable entering new homes and engaging with new patients./liliAs an IHA provider, you will be required to travel up to a 50-mile radius within your hired area.You are reimbursed for mileage greater than 30 miles in one day at the current government rate. Member outreach works to cluster your appointments in one geographical area as much as possible./liliCan sign up for a minimum of 30 appointment slots or more per month./li/ulpstrongGeneral Physical Demands:/strong/pulliAbility to lift 20 lbs./liliAbility to bend or kneel for extended periods of time if needed while performing assessments./li/ulpstrongEducation and Experience:/strong/pulliMaster’s or Doctorate degree from an accredited NP or PA program./liliHome health experience (Preferred)/lili1 year of in-home assessment experience (Preferred)/li/ulpstrongCertification and Licensure:/strong/pulliCurrent RN and NP or PA licensure in the present state of practice./liliBoard certifications accepted: FNP, ANP, AGNP, ACNP, AGACNP, and AGPCNP./liliCurrent BLS certification/liliValid unencumbered state driver’s license/liliValid proof of sufficient car insurance coverage for the state appointments are scheduled to be completed in./li/ulpstrongAbout Emcara Health:/strong/ppEmcara Health is reimagining how healthcare is delivered.
