Join our mission of offering hope, respect and support to our clients on their journey to mental health and wellness./p pstrongGENERAL PURPOSE/strong/p pResponsible for ensuring that clients are introduced into the Program in a comfortable, thorough and timely manner.nbsp; Completes the initial assessment of clients to confirm eligibility under target population; keeps clients and families engaged in services until assignment to primary therapist is successful./p pstrongESSENTIAL DUTIES AND RESPONSIBILITIES/strongbr / br / The duties listed below are intended only as illustrations of the various types of work that could be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to this class./p ul liResponsible for completing the intake Core Assessment, including collateral assessments (CANS, CHQ, MSE) in the field or in the office./li liResponsible for supporting the development of the Individual Client Plan in collaboration with the member and the treatment team through linkage and handoff at CFT.nbsp;/li liIdentifies strengths and challenges of child and family systemnbsp;/li liScreens for barriers to treatment.nbsp;/li liIdentifies child and familyrsquo;s natural supports./li liInteracts productively with the treatment team in order to provide holistic assessment and engagement services that are member driven and recovery oriented.nbsp;/li liCompletes all required paperwork as assigned in a timely manner.nbsp;/li liProvides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.nbsp;/li liProvides ldquo;on-the-spotrdquo; counseling that is both helpful to the clients and consistent with the philosophy of the program.nbsp;/li liInitiates contact with previous provider, Primary Care Physician, Psychiatric Hospital and/or collateral resources, to obtain assessment information and provide case management during transition as necessary.nbsp;nbsp;/li liEnsures clientsnbsp;discharging from psychiatric hospital are linked to agency Psychiatrist within 30 days. Ensures clientsnbsp;transferring to services with medication already in place are linked to agency psychiatrist.nbsp;/li liDevelops initial safety plan and monitorrsquo;s safety planning in high risk cases during assessment period.nbsp;/li liCompletes Service Requests and links clients and families to community resources as appropriate.nbsp;nbsp;/li liFacilitates groups and provide individual and family therapy.nbsp;/li liSupports Family Advocate with engagement of new referrals to program.nbsp;nbsp;/li liEnsures the safety, health, and well-being of the members.nbsp;/li liAdheres to and upholds the policies and procedures of Turning Point Community Programs.nbsp;/li liAttends all team and agency staff meetings unless approval for non-attendance is secured by the Program Director.nbsp;/li liMeets the standards set for performance in all aspects of job duties.nbsp;/li liMeets the standard for consistent attendance (not more than a 5% absence rate per month) by reporting to every scheduled shift on time and ready to work and by the judicious use of paid sick leave and avoiding any situations resulting in the need to dock pay for time not worked.nbsp;/li /ul pnbsp;/p pstrongSchedule:nbsp;/strongMonday ndash; Friday, 9:30 am - 6:00 pm/p p50% Virtual position. Would be required to attend standing on-site meetings (bi-monthly staff meetings, weekly group supervision, other required trainings as needed).
