Evaluates and treats patients, communicates with families, physicians and other interdisciplinary team members, documents services in the medical records. Demonstrates safe working practices./p pstrongProvides patient treatments which meet patient needs and uses current treatment knowledge in accordance with the facility clinical procedures. This includes but is not limited to:/strong/p ul liPerforming accurate, comprehensive patient evaluations./li liDeveloping effective plan of treatment and obtaining appropriate approvals from referring physician./li liSupervising Physical Therapy Assistants/other Physical Therapists and/or Aides in direct patient care and patient related activities according to Facility Policy as well as State and Federal requirements./li liCommunicating with supervisor and interdisciplinary team members regarding patient progress, problems, and treatment plans./li liInstructing patientrsquo;s families and/or nursing staff in specified follow through programs./li liInterviewing patients and families regarding previous level of functioning, life style and current and future expectations for recovery./li liDemonstrating knowledge of appropriate resources for problem-solving and clinical best practice./li /ul pstrongWrites accurate, complete, and clear documentation in accordance with regulatory, licensing, payor, and accrediting requirements. This includes but is not limited to:/strong/p pRecording Resident Needs Reviews, evaluations, daily treatment notes, progress notes, and discharge summaries in accordance with facility Policies and Procedures./p pRecording treatment charges accurately and insuring that documentation is consistent with billing data./p pWriting specific, objective documentation that is outcome oriented and details the patientrsquo;s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals./p pManaging Medicare Medical Review and Denials process in conjunction with Director of Rehabilitation, the facility, and therapy staff./p pAcquiring and documenting consents/approvals for treatment when necessary./p pstrongConsistently demonstrates facilityrsquo;s interdisciplinary approach to patient care. This includes but is not limited to:/strong/p pIntegrating findings of other members of the interdisciplinary team in treatment assessment and recommendations./p pIntegrating input from patient and family in treatment assessment and recommendations./p pPreparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions./p pCollaborating with and informing facility staff in areas of patient progress./p pEducating members of the team in areas of expertise./p pWorking with facility to teach documentation practices which assist the therapy team./p pCommunicating effectively with discharge planners./p pstrongContributes to professional development of self and others. /strongThis includes but is not limited tostrong:/strong/p pUtilizing continuing education to achieve targeted quality improvements./p pInteracting with peer therapist at local acute hospitals and other environments, if appropriate./p pPresenting at weekly rehab team conferences, facility in-services, and professional conferences./p pMaintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers./p pMeeting continuing education requirements of regulatory agencies./p pstrongAids in building and maintaining a positive facility culture as a professional member of the staff. /strongThis includes but is not limited to:/p pConsistently demonstrating positive core values in interaction with co-workers, patients and families, with all other persons involved with the patientrsquo;s care, and the general public./p pConsistently demonstrating professional standards./p pContributing to a positive work team by sharing information, using problem-solving methods and accepting new ideas, constructive criticism, or advice from others./p pDemonstrating superior customer service through courtesy, consistent follow-up and positive communication./p pstrongDemonstrates safe working practices. /strong This includes but is not limited tostrong:/strong/p pIdentifying and correcting safety hazards or notifying the facility or the Director of Rehabilitation so that safety hazards will be immediately remedied./p pMaintaining work areas in safe and orderly fashion./p pMaintaining equipment in safe working order by facilitation annual calibration./p pUsing proper body mechanics during treatments of patients and during support activities./p pUsing universal safety precautions./p pWearing appropriate protective equipment./p pstronguQUALIFICATIONS/u:/strong/p pstrongSkills, Knowledge and Abilities:/strong/p ul liKnowledge in Physical Therapy evaluation and treatments./li liAbility to supervise and direct assistants and aides./li liUnderstanding of the principles of physical modalities employed in Physical Therapy./li liKnowledge of use of prosthetic and orthotic devices ambulatory aides, assistive and resistive devices, and training./li liKnowledge of medical terminology./li liAbility to work with all types and levels of patients./li liMust have excellent verbal and written communication skills./li /ul pstrongEducation:/strong/p pBachelorrsquo;s Degree, Masterrsquo;s Degree, or Doctorate Degree in Physical Therapy from an accredited Physical Therapy program./p pstrongExperience:/strong/p pExperience in geriatric rehabilitation preferred./p pExperience in adult neurological, cardiac, and/or general rehabilitation is preferred./p pstrongPhysical Requirements:/strong/p pstrongFor Direct Patient Care:/strong/p pMust be able to lift and transfer patients to and from all surfaces a minimum of 10 times per day./p pMust be able to lift approximately 50-70 pounds a minimum of 10 times per day./p pMust be able to perform CPR, if required./p pMust be proficient with use of hands for repetitive grasping, pushing-pulling, and fine motor manipulation./p pPhysical Therapist will spend approximately 60-80% of time bending, stooping, kneeling, and reaching while working with physical therapy patients./p pPhysical Therapist will spend approximately 60-80% of time standing/walking./p pstrongFor Support Activities:/strong/p pPhysical Therapist will spend approximately 10-20% of time sitting at desk or terminal entering data./p pstrongLicense/Certification/Registration:/strong/p pCurrent Physical Therapist Registration/State Licensure as required/p pCurrent CPR certification./p
