Care Coordinator Supervisor (LMSW/LCSW/LMHC/LMFT/RN)

Title: Care Coordinator Supervisor – Health Home (HHSC)
Pay Range: $32.69 - $34.06/Hr. Based on experience

The Care Coordinator Supervisor will be responsible for the supervision of Care Coordinators operations within the Health Home Servicing Children (HHSC) division. The HHSC Care Coordinator Supervisor participates and interacts with all staff in a supportive role as it relates to care management and coordination daily operations.

Qualifications required:

• At least 3 years previous managed care experience.
• Ideally 2 years specific to Behavioral Health/Children Social Services.
• Bachelor’s degree in human services field (i.e., counseling, education, nursing, psychology, social work, etc.)
• MSW/MA/MS Master’s Degree, preferred.
• LMSW, LCSW, LMHC, LMFT, RN preferred.
• Two years’ experience working with Behavioral Health and/or Substance abuse required.
• Prior supervisory experience required.
• Knowledge of the Collaborative Care Model.
• Experience with chronic condition management, particularly Diabetes, HIV, Heart Disease
