JOB SUMMARY 
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible 
and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic 
barriers to healthcare and promote health equity for all. We offer a wide range of services, including 
mental health care, residential and outpatient substance use treatment, and primary health services. 
Additionally, we provide transitional support for individuals re-entering the community after involvement 
in the criminal justice system. By integrating physical and behavioral health, we empower individuals to 
overcome challenges by addressing social determinants of health, fostering resilience, and facilitating 
recovery.


Prototypes is committed to creating healthier families and stronger communities through a wide range of 
prevention, education, and treatment services. Our outpatient substance use disorder programs for adults 
provide evidence-based care tailored to individual needs, including specialized support for parents, 
pregnant women, and those involved with child welfare or the court system. We also offer comprehensive 
behavioral health services for children (ages 0–20) and adults (21+), ranging from early intervention to 
intensive mental health treatment. For youth, we provide programs that promote alcohol- and drug-free 
lifestyles, while supporting parents in strengthening their families and preventing child abuse. Through 
counseling, therapy, psychiatric care, medication management, and coordinated holistic support, our 
dedicated team helps every client build a healthier, happier, and more resilient life.


The Case Manager I delivers client services by coordinating care within the organization's continuum of 
services and with external human services providers. They collaborate closely with clients to assist in 
completing intake paperwork, outcome assessments, and applying for benefits. Working alongside a 
multidisciplinary team, Case Manager I ensures optimal treatment outcomes aligned with HealthRIGHT 
360's philosophy, goals, policies, mission, and vision. Reliable transportation is required as our services 
are provided flexibly across outpatient offices, residential treatment settings, or in the community as 
dictated by client needs and program requirements.


KEY RESPONSIBILITIES 
Leadership
• Serve as a role model for staff, demonstrating best practices in service delivery and professional 
conduct.
• Provide guidance, support, and coaching to team members to enhance performance and client 
outcomes.
• Promote a safe and healthy work environment, ensuring compliance with environmental, safety, 
and health standards.
• Foster a culture of accountability by ensuring team adherence to organizational policies and 
procedures.
• Ensure all client documentation is accurate, complete, and submitted on time according to 
program, regulatory, and organizational requirements.
• Monitor staff compliance with policies, procedures, and performance expectations.
• Support adherence to regulatory, contractual, and program-specific requirements as delegated 
by management.
Case Management and Client Support
• Complete case management assessment and enter financial and benefit information during client 
admission.
• Monitor client's progress towards treatment plan goals and provide input.
• Support client in Medi-Cal application or transfer.
• Assess client’s case management needs and complete releases of information.
• Connect clients to various services: benefits, healthcare, employment, housing, outpatient 
substance use services, etc.
• Coordinate external service linkage and communication (appointments, probation, child visits, 
court minute orders, etc.).
• Complete VI-SPDAT and connect client to housing options.
• Collaborate with clients on rehabilitation goals.
• Prepare clients for job search and aid with resume writing.
• Provide treatment oversight for case management services.
• Provide translation services as needed.
• Complete client UA tests and documents results (if assigned).
• Transport clients as needed.
Child Specific Duties
• Provide support for parents and caregivers to strengthen the family members’ network of 
relationships and community resources.
• Provide field-based services as required by the program.
• Collaborate with school staff, and advocate and participate in IEP or 504 plan meetings as needed.
FSP-Specific Duties
• Complete and submit rent invoices/ flex fund request/ and vendor check requests as needed. 
• Complete KEC/OMA’s at scheduled due date. 
• Participate in Outreach and Engagement Process (including commuting into the field as needed).
CRP-Specific Duties
• Outreach and engagement for referred individuals and clients struggling to meet probation 
mandates (Including commuting into the field as needed). 
• Complete weekly outreach and engagement reports. 
• Provide jail in-reach and post-release case management as needed. (Services may take place in 
the office, jails, and probation hubs).
• Collaborate with clients and their families to support reintegration into the community.
• Coordinate and collaborate with law enforcement to promote compliance and increase the 
likelihood of success of the client.
• Assist with the development of groups and activities that meet the needs of the clients served. 
• Offer peer support to clients when needed to obtain needed community resources.
• Collaborate with outside agencies to help clients obtain needed community resources (housing, 
sober livings, substance abuse recovery agencies, medical facilities, employment/education 
agencies, DPSS, DMV or other needed resources).
Documentation
• Write progress notes within 24 hours of service delivery.
• Write client progress letters and court reports with supervisor approval.
• Maintain compliance with clinical documentation standards (HIPAA, 42-CFR, DMH, SAPC).
• Complete Peer Reviews (Chart Audits) in a timely manner.
Administration and Other Duties
• Seek clinical consultation in new or high-risk situations.
• Maintain expected client care hours and productivity.
• Attend and actively participate in assigned Clinical Supervision.
• Participate in agency, team, and assigned meetings.
• Participate in training opportunities and complete assigned training.
• Communicate collaboratively with all members of the behavioral health team.
• Arrange work schedule according to program needs.
• Read and respond to emails daily.
• Maintain compliance with agency policies and procedures.
• Maintain compliance with all relevant standards and regulations.
And perform other duties as assigned. 


QUALIFICATIONS
Credentials and Experience
• Option I:
o Bachelor’s degree from an accredited university.
o Registration as an SUD Counselor from CAADE, CCAPP or CADTP.
• Option II:
o High school diploma or equivalent.
o Certification as an SUD Counselor from CAADE, CCAPP, or CADTP.
• Two years’ case management experience.
• Experience working with clients experiencing acute withdrawal from substances.
• Experience in the human service field and demonstrated expertise in substance abuse treatment, 
relapse prevention, and recovery.
• Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
• Valid California Driver’s License and access to registered and insured transportation.
