pWe are looking for a compassionate Hospice Licensed Practical Nurse (LPN) to join our team. Reporting directly to the Administrator or Executive Director, you will play a vital role in delivering hands-on hospice care to patients and families in their homes, assisted living, or facility settings. pstrongAs a Hospice LPN, You Will:/strong/p
ul
lipProvide direct nursing care services to hospice patients, supporting both patients and their families./p/li
lipAssist the interdisciplinary team in assessing patient/family needs and contribute to the execution of individualized care plans./p/li
lipReport any changes in patient conditions promptly to the Registered Nurse and care team./p/li
lipDeliver empathetic care aligned with each patient’s unique plan of care./p/li
lipServe as a trusted resource to families, offering support during difficult times./p/li

pstrongResponsibilities:/strong/p
ul
lipProvide skilled bedside nursing care under the supervision of a Registered Nurse, following established nursing protocols and the Nurse Practice Act./p/li
lipMonitor patients actively and document any changes in condition, reporting as necessary./p/li
lipAdminister medications and treatments according to the plan of care./p/li
lipParticipate in infection control practices and standard precautions./p/li
lipSupport bereavement activities and closure contacts with families as assigned./p/li
lipParticipate in on-call duties and respond to patient needs during scheduled visits or emergencies./p/li
lipMay assist with death pronouncements per state law and under RN supervision./p/li
lipCollaborate closely with hospice aides and other team members to ensure comprehensive care delivery./p/li

pstrongQualifications:/strong/p
ul
lipGraduate of an accredited Practical Nursing program./p/li
lipCurrent LPN/LVN license in the state of employment./p/li
lipMinimum 2 years of LPN experience in hospice, oncology, long-term care, or home health; 3-5 years preferred./p/li
lipCPR certification required./p/li
lipValid driver’s license and current automobile insurance./p/li

lipUnderstanding of hospice care principles and the unique needs of terminally ill patients and their families./p/li
lipExcellent patient assessment and clinical skills./p/li
lipStrong communication and documentation abilities./p/li
lipFamiliarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards./p/li
lipAbility to work independently and as part of an interdisciplinary team./p/li
