• Must have updated work history with no gaps exceeding 30 days
• Physician statement required for any malpractice history
• BLS required
• Must provide email and phone number with submission

Requirements:

• Active New York License (License in Hand Required)
• BC Urologist (Board Certified Required)
• Epic Experience Required
