Requirements:

• Active LVN/LPN license in the practicing state
• Graduate of an accredited LVN/LPN program
• BLS certification
• Skilled Nursing or Long-Term Care experience (preferred)
• Knowledge of state nursing regulations and SNF protocols
• Ability to lift, reposition, and assist residents as needed

Responsibilities:

• Provide direct patient care according to care plans and physician orders
• Administer medications and treatments as prescribed
• Monitor and document patient conditions, vital signs, and changes in status
• Assist with admissions, discharges, and transfers
• Perform wound care and assist with rehabilitative services
• Collaborate with RNs, physicians, and interdisciplinary care teams
• Educate residents and families on care plans and treatments
• Maintain accurate and timely documentation
• Follow facility policies, state regulations, and infection control standards
