MS: 1081594 Minimum 2 years of experience ARRT, BLS Certification License required to submit Shift Notes: 4 x 10 hour shifts with call on weekend and nights CS: Epic Unit Notes: Local Rate: Applies within 50 miles (–$25 on regular & OT rates) Call-Off Policy: Up to 12 hours per 13 weeks (no penalty) Orientation: 12 non-billable hours (guaranteed hours not applicable) Submission Requirements: Last 4 digits of SSN Middle initial Requested time off (RTO) Active IL license or proof of application Must comply with drug-free campus policy (negative drug screen required)
