Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in skilled nursing or long-term care settings preferred.
• Minimum of one year of clinical experience in physical therapy preferred.
• Experience with patient documentation systems and electronic medical records preferred.

• Work Schedule: 5x8.
