Minimum Requirements

  • Master's or Doctoral Degree in Physical Therapy from an accredited program (MPT or DPT required).
  • Active Colorado PT license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home Health OASIS Clinician experience required.
  • Ability to provide services across a coverage area up to a 50 mile radius of Fort Morgan.
  • Experience with home-based skilled PT interventions including therapeutic exercise, gait and balance training, transfer training, functional mobility, pain management, and fall prevention.
  • Experience completing required home health documentation (OASIS components, visit notes, reassessments, discharge summaries) and communicating with interdisciplinary care teams.
  • Preferred: HCHB (Homecare Homebase) electronic documentation experience.

  • Shift name: Days.
  • Shift schedule: Monday–Friday, 0800–1600 (days).
  • Weekly hours: 40.
  • Productivity expectation: 30 points.
  • Coverage area: Up to 50 mile radius of Fort Morgan; mileage reimbursement provided between visits from first patient to last patient.
  • Certification required: MPT or DPT.
  • Minimum education requirement: Master’s degree.
  • On call/weekend requirement: None.
