About the Role:
Join Bellestar Home Health Care Inc as a Home Health PTA in Roseville, CA, where you will play a vital role in enhancing the quality of life for our patients. We are looking for passionate and dedicated professionals to support our mission of providing exceptional home health care services.

Responsibilities:

• Assist patients with physical therapy exercises and rehabilitation programs.
• Monitor patient progress and report changes to the supervising physical therapist.
• Educate patients and families on home exercise programs and safety techniques.
• Maintain accurate documentation of patient care and progress notes.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Ensure compliance with health care regulations and company policies.
• Provide compassionate support and encouragement to patients during therapy sessions.
• Travel to patients' homes within the service area as required.

Requirements:

• Active Physical Therapist Assistant (PTA) license in California.
• Experience in a home health care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of physical therapy principles and practices.
• Current CPR and First Aid certification.
• Reliable transportation and valid driver's license.
• Compassionate demeanor with a patient-centered approach.
