Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient vital signs and report any changes to the healthcare team.
• Assist in daily living activities, ensuring patient comfort and safety.
• Document patient care and progress accurately in electronic health records.
• Collaborate with registered nurses and physicians to develop and implement patient care plans.
• Educate patients and families on health management and disease prevention.
• Maintain a clean and organized environment in accordance with healthcare regulations.
• Participate in staff meetings and training sessions to enhance professional development.

Requirements:

• Valid LPN license in the state of Maryland.
• Minimum of 1 year of experience in a healthcare setting, preferably in home health or long-term care.
• Strong communication skills and a compassionate demeanor.
• Ability to work independently and as part of a collaborative team.
• Basic Life Support (BLS) certification required.
• Proficient in electronic health record (EHR) systems.
• Commitment to providing high-quality patient care and support.
• Flexibility to work various shifts, including evenings and weekends.
