COME JOIN OUR Independent Living Services TEAM! Supporting individuals in San Marcos, Escondido, Vista and Oceanside areas.


Schedule shows a range of hours- 8 hours must be worked.
Monday 8:00am-8:00pm
Tuesday 8:00am-8:00pm
Wednesday 6:30am- 6:30pm
Thursday 8:00am-8:00pm
Friday 8:00am-8:00pm
Saturday (occasional) 8:00am-8:00pm

Join Spread Your Wings, LLC. – ILS Services as a Life Skills Mentor/Coach and support individuals with developmental disabilities in achieving greater independence and confidence in their daily lives.


What you'll do:

  • Coach clients on building independent living skills
  • Spend 90% of your time in the field with clients and 10% on admin/documentation
  • Assist, monitor, and teach individual(s) with preparation of daytime activities and encourage good personal hygiene and behavior appropriate in their home or out in the community. These activities may include grocery shopping, menu planning, meal preparation, and clean up.
  • Encourage, motivate, and support clients with developmental disabilities and/or behavioral challenges.
  • Help plan, create, and organize tasks and schedules that promote goal achievement for your clients.
  • Provide consistent documentation of progress through case notes and quarterly reports.

Requirements:

  • Valid driver’s license, clean driving record, and reliable transportation
  • Able to work and commute in and around the assigned Orange County area
  • Able to pass a background check (per state industry standards)
  • Computer literacy: MS Word, email, basic data entry

Requirements:

Primary Responsibilities

  • Ensures quality, health, and safety with client centered approaches for individuals in community living services.
  • Ensures compliance with federal, state, and local regulations while expanding opportunities for client(s) serviced to participate in community life as they choose.
  • Must be knowledgeable and keep abreast of Individual Support Plan (ISP) and Individual Placement Plan (IPP) and assist with the development and implementation
  • Assessment, planning, implementation, and review of client centered support plans for client(s) served.
  • Coordinate the delivery of all services provided by Spread Your Wings and the community.
  • Assist client(s) in expanding their opportunities to participate in community life as they choose.
  • Assist with staff schedules and services identified on the Individual Support Plan (ISP) and responding to emergency crisis.
  • Responsible for client home visit(s)/check(s) and client inquires.
  • Facilitate wellness check(s) at client’s homes for food, safety, hygiene, and resources.
  • Collect all SYW documents and submit them to the office for filing.
  • Respond to client emergencies while out in the field and to the emergency phone.
  • Completion of Special Incident Report (SIR) when needed and submit to Regional Center within 24 hours of incident.
  • Report all client incident(s) to Office Case Manager (OCM) and Program Manager (PM).
  • Attend meetings with clients.
  • Maintains a prominent level of confidentiality.
  • Performs other duties as assigned.

Qualifications

Experience: 1 year of People Management experience

Knowledge/Skills: Organization, interpersonal, communication and problem-solving skills.
