Community Home Health Care is hiring a Home Health Physical Therapist (PT) to provide skilled therapy services to patients in their homes. This role allows you to work independently while helping patients improve mobility, regain independence, and safely remain at home.

 Responsibilities

• Evaluate patients and develop individualized plans of care

• Provide skilled physical therapy treatment in the home

• Monitor patient progress and adjust treatment plans as needed

• Educate patients and families on exercises and safety

• Maintain accurate clinical documentation

• Collaborate with the healthcare team to support patient outcomes

 Requirements

• Graduate of an accredited Physical Therapy program

• Active Physical Therapist license

• 1+ year PT experience (home health preferred)

• Strong communication and clinical assessment skills

• Ability to travel to patient homes
