Taconic at Hopewell is seeking a compassionate and skilled Part-Time Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. Position Summary

The Speech-Language Pathologist provides evaluation, diagnosis, and treatment of speech, language, cognitive, and swallowing disorders for residents in a skilled nursing and rehabilitation setting. The SLP collaborates with the care team to improve safety, independence, and quality of life.

Key Responsibilities
  • Evaluate and treat residents with speech, language, cognitive, and swallowing disorders

  • Develop and implement individualized care plans

  • Perform dysphagia assessments and recommend safe dietary modifications

  • Collaborate with nursing, dietary, and rehabilitation teams

  • Educate residents, families, and staff on communication and swallowing strategies

  • Maintain accurate documentation in accordance with regulatory standards

  • Support quality measures and patient outcome goals

Qualifications
  • Degree in Speech-Language Pathology

  • Current SLP license (required)

  • Certificate of Clinical Competence (CCC-SLP) preferred

  • Experience in skilled nursing or post-acute care preferred

  • Strong clinical, communication, and documentation skills
