Supportive office staff who answer the phone and have your back
Local, accessible leadership — no corporate layers
Provide high-quality, skilled nursing care to patients in their homes
Administer medications and treatments as ordered
Perform assessments, monitor changes, and communicate updates to the care team
Educate patients and families on disease processes, medications, and self-care
Collaborate with RNs, therapists, physicians, and caregivers to support the care plan
Document accurately and timely according to state, federal, and agency standards
Ensure all patient care meets regulatory and clinical guidelines
Current LPN license in good standing (Oklahoma)
Home health experience preferred (we also welcome strong clinical LPNs ready to transition into home health)
Excellent communication and organizational skills
Ability to work independently with confidence and sound judgment
Passion for providing compassionate, patient-centered care
Reliable transportation, valid driver’s license, and current auto insurance
