Summary
This position is part of a Supportive Housing partnership between Simpson Housing Services, Aeon, andAvivo(formerly Resource, Inc. OR RESOURCE Chemical and Mental Health). The position will provide supportive services to residents at Aeons OHP properties,the Continental andthe Lamoreaux. OHP is a set of Aeon buildings thatprovidesformerly homeless single adults with a safe and permanent home and support services to help residents regain andmaintainstability in their lives,homesand the community. This energetic and adaptable employee will work as part of a team toassistresidentsto maintainhousing,supportaccess to community resources, and foster community engagement and resident leadership. The Resident Connections Coordinator (RCC) must be knowledgeable about and able to respond effectively to the complexities of participants lives and the co-occurring challenges and conditions associated with long-term homelessness.
The RCC will work as part of a teamcomprisedof staff from three different agency partners (Simpson, Aeon, andAvivo).Hours for this position are Mondays to Fridays, 9-5pm.
Responsibilities
Provide quality service to participants:
- Welcome new residents and actively build relationships, explaining the resident services program, and offering information and support to those wishing to access resources.
- Develop supportive professional relationships with residents that help enhance the quality of their lives,empowerand encourage them in taking the steps to achieve self-sufficiency.
- Identify,developandmaintainreferral partnerships with local social service agencies that effectivelyassistresidents to achieve their basic needs and life goals in areas including education, employment, mental and physical health. Accessing mental health crisis services or Adult Protective Services for residents is a commoncomponentof the position.
- Partner with Property Management when a resident is in jeopardy of eviction andoffersupport to the resident topositively and quickly rectify the situation.
- Provide flexible services during times that meetneedsof participants.
Provide opportunities for community engagement for residents:
- Facilitate resident meetings with Property Management and implement community engagement activities such as support groups, movie/game nights, resident leadership council, community health educationworkshopsand social celebrations,sometimeswith the support of outside volunteers.
- Provide support and leadership to team members to enhance community engagement at both the Continentaland the Lamoreaux
- Ensure residents are informed of opportunities toparticipatein the direction of programs, and that residents have bona fide and systematic access for expressing concerns and affecting positive change.
- Encourage andfacilitateresident leadership through the RPC(Resident Planning Committee)and other resident leadership opportunities.
Demonstrate teamwork and collaboration skills with 3 different agency partners:
- Communicate and coordinate services with all team members. Attend regular team meetings and other required meetings.
- Develop andmaintainpositive working relationships with staff, volunteers,participantsand other stakeholders.
- Network withother communityproviders of similar services
Completeaccurateand thorough documentation and recordkeepingin a timely manner:
- Maintain individualelectronicClient Trackfiles, includingtimelydetailedcase notes, signed data privacy, intake and discharge summaries, and Releases of Information.
- Understand andcomply withnecessary documentation for MN Housing and Hennepin CountyHousing Support.
- Maintainaccuraterecords including employee time sheets and travel expenses.
- Participate with funding reports, and site visits asrequested.
Demonstrate essential knowledge, abilities, and skills:
- Ability to work very well independently, with excellent skills in teamwork and collaboration.
- Ability to be flexible and manage time.
- Knowledge of Harm Reduction/Housing First Model and commitment to that model.
- Knowledge of trauma-informed service and motivational interviewing techniques.
- Keep up to date and knowledgeable on community resources and trends. Expand knowledge base around topics that affect OHP resident population.
- Ability to communicate effectively,positivelyand professionally with participants, providers, landlords, familymembersand others.
- Ability to promote a professional work environment that is affirming,respectfuland understanding of diverse people.
- Have organizational skills and be able to multi-taskin order toaccomplishthe day-to-day activities.
- Mustpossessgoodcomputer skills, with experience in Microsoft Word, Excel, and Outlook. Demonstrateabilityto learnadditionalcomputer skills/programas necessary.
- Ability tomaintainconfidentiality.
- Adhere to a standard framework of boundaries in all interactions with guests, volunteers,staffandpublic.

Job Requirements:
- Bachelorsdegree in human servicesand/or related field of study or equivalent experience isrequired.
- Experience working with individuals who are homelessor have experiencedhomelessness.
- Broad knowledge of housing resources.
- Drivers license and access to a car.
Desired:
- Experience working with adults with mental illnessand/or chemical dependency.
Essential Physical Functions
The employee must be able to hear and speak,in order tocommunicate with staff and participants in person, on the telephone and in writing.Employeemust be able to move about the community and public places (such as apartment buildings).Employeesmust be able to lift items up to25pounds on an occasional basis.
