Job Description
St. Patrick's Manor Nursing & Rosarie Center located in Framingham, MA has an exciting opportunity available for a Full-time and PRN Physical Therapist (PT) to join their new in-house therapy team!

Treatment programs are designed to meet the physical and emotional goals necessary for residents to reach their optimum levels of functioning. Key Responsibilities:
- Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.
- Deliver physical therapy services to restore mobility, reduce pain, and improve physical function and independence.
- Collaborate with an interdisciplinary care team including nurses, occupational therapists, speech therapists, and physicians.
- Educate patients and families on home exercise programs, safety strategies, and wellness practices.
- Supervise Physical Therapist Assistants (PTAs) and ensure adherence to treatment plans.
- Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
- Participate in care planning, team meetings, and quality improvement initiatives as needed.
Qualifications:
- Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
- Current State Physical Therapist license in good standing (or eligible).
- Current CPR certification (or willingness to obtain upon hire).
- Strong clinical, organizational, and interpersonal skills.
- Previous experience in a skilled nursing or long-term care setting is preferred but not required.
