Liberty Health Care Center is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team in Liberty, TX. We are committed to providing high-quality, patient-centered care in a skilled nursing setting, helping residents improve mobility, strength, and overall functional independence.
Our in-house therapy program promotes collaboration, continuity of care, and strong interdisciplinary teamwork.
Position Summary
As a Physical Therapist (PT), you will be responsible for evaluating residents, developing individualized treatment plans, and delivering skilled therapy services to maximize functional outcomes and quality of life.
Key Responsibilities
• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled PT services in accordance with physician orders and clinical standards
• Address mobility, balance, strength, gait training, and pain management
• Monitor resident progress and modify treatment plans as appropriate
• Educate residents and caregivers on exercises, safety, and functional mobility
• Complete accurate and timely documentation per facility, state, and federal guidelines
• Collaborate with the interdisciplinary team to support optimal resident outcomes
• Maintain compliance with all professional, ethical, and facility standards
Qualifications
• Active Texas Physical Therapist (PT) license
• In good standing with all regulatory and licensing boards
• SNF or long-term care experience preferred
• Familiarity with Net Health is a plus
• Strong clinical, communication, and time-management skills
