Physical Therapist Per Diem

We are seeking a licensed Physical Therapist (Per Diem) to join our growing healthcare team. This role is ideal for a self-motivated clinician looking for flexible, as-needed work while providing high-quality, patient-centered care.

Responsibilities:

• Perform patient evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, manual therapy, and patient education
• Monitor patient progress and modify care plans as needed
• Complete accurate and timely clinical documentation
• Communicate effectively with patients, families, and care team members
• Comply with all healthcare, safety, and ethical standards

Qualifications:

• Active Physical Therapist license
• Ability to work independently and manage time efficiently
• Strong communication and interpersonal skills
• Reliable transportation (for home health visits, if applicable)
• Home health experience preferred but not required

Position Details:

• Job Type: Per Diem
• Schedule: Flexible / As needed

Pay (Per Visit):

• Physical Therapy Evaluation: $90 per evaluation
• Physical Therapy Visit: $65$75 per visit
