Home Health Physical Therapist

Description:

We are looking for a Physical Therapist to join our home health team in the Naples, Florida area. ACT Physical Therapy is a rehabilitation group consisting of PT's, OT's, PTA's, and COTA's and we provide rehabilitation services for agencies in Southwest Florida. With the contractor option, we have no point or productivity requirements so the workload is at the PT’s discretion. Visits performed will be evaluations, treatments, reassessments, discharges, and recertifications. Experience in the home health setting is preferred but not required. Documentation is with Pointcare (Homecare Homebase) which will require an Android based tablet. Training is available in North Port, FL if needed but it is not required for applicants familiar with the documentation system.

Requirements:

Active Florida License to practice Physical Therapy

Florida Driver's License

Recent PPD

Professional Liability Insurance

Current CPR (taken in person)

HIV for healthcare professional's course

Alzheimer's course that is DOEA approved
