 Full-time days. 12 hr shifts. Pay rate $35.00 per hour. 

Job Description
Our Primary Care team is looking for an exceptional nurse to help us manage and coordinate care for our high-acuity patients. Our nurses enjoy strong relationships with patients, collaborative work environments and a deep sense of belonging and appreciation. Nurses are responsible for carrying out a variety of healthcare services, including:

● Medication management;
● Coordinating care between outside providers and hospital systems;
● Disease management;
● Wound care;
● Recording vitals;
● Developing Plans of Care;
● Phlebotomy;
● Hydration Therapy and more!

Qualifications:
● Active Driver’s License
● Active LPN License
● BLS/CPR Certification
