Job Description
Position Summary
The Physical Therapist (PT) is responsible for providing physical therapy services in accordance with state and federal regulations, professional standards, and Add to Life's policies and procedures. The PT evaluates, develops, implements, and modifies treatment plans that are designed to restore functional capabilities, improve mobility, relieve pain, and prevent or limit permanent physical disabilities for patients receiving home health services.
Qualifications
Current licensure as a Physical Therapist in the State of Wisconsin
Graduate of an accredited Physical Therapy program
Minimum of one (1) year experience in physical therapy, home health experience preferred
Current CPR certification
Valid driver's license and reliable transportation with proof of auto insurance
Excellent communication and interpersonal skills
Ability to work independently and as part of an interdisciplinary team
Responsibilities
1. Patient Care
a. Perform comprehensive physical therapy evaluations for all assigned patients
b. Develop individualized care plans based on evaluation findings and patient needs
c. Provide direct physical therapy interventions following the established plan of care
d. Observe, record, and report patient status and response to treatment
e. Modify treatment plans as needed based on patient progress and response
f. Educate patients and caregivers on home exercise programs, safety, and mobility techniques
g. Coordinate care with other disciplines to optimize patient outcomes
2. Documentation and Compliance
a. Complete all required documentation accurately and timely in accordance with Medicare/Medicaid guidelines and agency policies
b. Document all patient encounters, interventions, and outcomes per ACHC standards
c. Ensure all documentation meets requirements of WI DHS Admin Code 106 & 133
d. Maintain patient confidentiality in accordance with HIPAA regulations
e. Participate in chart audits and quality improvement initiatives
f. Adhere to all regulations outlined in CFR 484 for home health services
3. Professional Standards
a. Maintain current licensure, certifications, and continuing education requirements
b. Attend and participate in staff meetings, in-service training, and professional development opportunities
c. Stay current with evidence-based practices in physical therapy
d. Adhere to APTA Code of Ethics and Standards of Practice
e. Participate in performance improvement activities and QAPI program
4. Administrative Duties
a. Manage caseload efficiently while maintaining high quality of care
b. Collaborate with Healthcare Administrator and Clinical Manager on scheduling and resource allocation
c. Submit accurate time sheets, mileage logs, and other required administrative reports
d. Participate in on-call rotation as assigned
e. Report equipment needs and maintenance issues promptly
Working Conditions
Travel to patients' homes in various settings and communities
Ability to lift, transfer, and position patients safely
Exposure to various home environments and potential infectious conditions
Flexible scheduling to accommodate patient needs
Occasional evening and weekend responsibilities
Supervision
Reports directly to the Healthcare Administrator/Clinical Manager
