PRN Physical Therapist (PT) Skilled Nursing Facilities (SNFs)
Location: Near Winamac, Indiana (2 Facilities)

A growing healthcare organization is seeking a dedicated Physical Therapist (PT) to provide evaluation services across two skilled nursing facilities located near Winamac, IN. This role offers the opportunity to work with a collaborative interdisciplinary team while making a meaningful impact on residents quality of life.

Position Summary:
The Physical Therapist will be responsible for completing evaluations, developing individualized treatment plans, and supporting rehabilitation goals for residents in a long-term care setting. This role focuses heavily on assessments and care planning, ensuring each patient receives the appropriate level of therapy services.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations for new and existing residents
  
  
• Develop and implement individualized treatment plans based on patient needs
  
  
• Collaborate with therapy staff, nursing, and physicians to ensure coordinated care
  
  
• Monitor patient progress and adjust plans as needed
  
  
• Maintain accurate and timely documentation in accordance with regulatory standards
  
  
• Provide guidance and support to therapy assistants as appropriate
  
  

Qualifications:

• Licensed Physical Therapist in the state of Indiana (or eligible)
  
  
• Previous experience in skilled nursing or long-term care preferred
  
  
• Strong clinical assessment and documentation skills
  
  
• Excellent communication and teamwork abilities
  
  
• Ability to manage time effectively across multiple locations
