Licensed Practical Nurse

Multicare Home Health Services, Inc. is seeking Licensed Practical Nurses to join our growing, family-owned agency. For over 27 years, we have been dedicated to providing compassionate, high-quality care to our clients and their families—and we’re looking for nurses who share that commitment.

We are seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). This role is ideal for a nurse who values patient-centered care and enjoys working independently while remaining part of a collaborative healthcare team.

Responsibilities:

• Provide skilled nursing care in accordance with physician orders and care plans
• Observe and assess patients’ overall health and report changes or concerns to physicians and supervising RN
• Administer medications via various methods, including injections, as ordered
• Monitor vital signs and document patient status accurately
• Educate patients and caregivers on medication management, wound care, and nutritional needs
• Communicate effectively with patients, families, and interdisciplinary care team members
• Ensure compliance with all clinical and documentation standards

Requirements:

• Active Licensed Practical Nurse (LPN) license (required)
• Current Basic Life Support (BLS) certification
• Updated TB test or chest X-ray
• Valid government-issued ID
• Strong basic computer skills for documentation
• Must have a reliable smartphone for communication and documentation purposes
• Excellent communication and observational skills
• Ability to work independently in a home care setting

Preferred Qualifications:

• Previous home health or skilled nursing experience
• Strong organizational and time management skills
