Overview
Continuum Home Health LLC is a nurse-led home health agency providing compassionate, patient-focused care throughout the Houston region. Duties
- Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
- Develop and implement individualized care plans tailored to each patient’s needs, goals, and medical history.
- Deliver targeted physiotherapy interventions such as trigger point therapy, orthopedics-focused treatments, and geriatrics care to optimize recovery outcomes.
- Manage discharge planning processes by coordinating with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care.
- Educate patients and their families on exercises, injury prevention strategies, and physiologic concepts to promote self-management.
- Document all patient interactions accurately using Electronic Medical Record (EMR) systems, maintaining detailed records of progress and treatment adjustments.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care across acute care, hospital, outpatient clinics, and community settings.
Experience
- Experience in home health or community-based care preferred.
- Background in inpatient or hospital settings beneficial for acute care decision-making.
- Outpatient/clinic experience in orthopedics, geriatrics, and general rehabilitation strongly valued.
- Strong knowledge of anatomy, physiology, and exercise prescription for patient-specific treatment planning.
- Experience with discharge planning and care coordination for hospital-to-home transitions.
- Proficiency with EMR documentation (Winnolas EMR training provided).
- Ability to deliver safe, effective therapy across diverse patient populations while maintaining evidence-based practice.
