Position Overview
As a Per Diem Physical Therapist, you will provide skilled home health PT services to patients across our
service areas. You will conduct evaluations, develop individualized treatment plans, deliver hands-on
therapy, and collaborate with our interdisciplinary care team — all while making a real difference in
patients daily lives.
Key Responsibilities
✓ Conduct comprehensive PT evaluations and develop individualized treatment plans
✓ Deliver skilled PT interventions including therapeutic exercise, manual therapy, gait training, and
fall prevention
✓ Educate patients and caregivers on home exercise programs and safe mobility
✓ Complete accurate, timely clinical documentation per Medicare/Medicaid standards
✓ Collaborate with physicians, nurses, OTs, and other care team members
✓ Participate in care conferences and discharge planning as needed
Qualifications
✓ Active, unrestricted Physical Therapist (PT) license in good standing
✓ Graduate of an accredited PT program (DPT or MPT)

✓ Minimum 1 year of PT experience (home health experience a strong plus)
✓ Valid driver&# license and reliable personal transportation
✓ Strong clinical documentation and communication skills
✓ CPR/BLS certified (or willing to obtain prior to start)
Preferred Experience
✓ Home health or geriatric PT background
✓ OASIS certification or experience with OASIS-E assessments
✓ Experience with orthopedic, neurological, or post-surgical populations
Compensation Perks
✓ $90–$120 per visit (based on visit type and experience)
✓ Additional compensation available for evaluations and specialized visits
✓ Mileage reimbursement between patient visits
✓ Flexible PRN schedule — work as much or as little as youd like
