Salary: $54,587.52 to $77,819.04 Annually plus benefits
Sign On Bonus: $5,000
Shift Information: 3, 12 hour shifts, working nights
Overview:
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. The ideal candidate will possess strong clinical skills, a thorough understanding of medical procedures, and a commitment to enhancing patient well-being. This position offers an opportunity to work with diverse patient populations, including individuals with developmental disabilities, dementia, and acute medical needs.
Duties:
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions.
- Administer medications and injections accurately, including IV infusions and other specialized treatments.
- Assist with complex procedures such as catheterization, wound suturing, and respiratory management including ventilator support.
- Maintain detailed and accurate medical documentation within Electronic Health Record (EHR) systems such as Epic, Cerner, or Athenahealth.
- Support discharge planning and coordinate care transitions for patients in inpatient or outpatient settings.
- Conduct triage assessments over the phone or in person to determine urgency and appropriate level of care.
- Provide specialized care for patients with Alzheimer's disease, dementia, or in hospice settings while adhering to HIPAA regulations.
- Collaborate with multidisciplinary teams including physicians, case managers, therapists, and family members to optimize patient outcomes.
- Demonstrate proficiency in handling emergency situations such as acute pain management or trauma response within Level I or Level II trauma centers.
- Engage in ongoing education related to ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and infection control protocols.
Experience:
- At least one (1) year experience as LVN
Education:
- Current California LVN license.
- Graduate from an approved LVN program with an associates or equivalent degree.
- Current CPR Certification.
Knowledge, Skills, and Abilities:
- Ability to apply good clinical judgment, plan work, establish priorities and maintain flexibility.
- Proficient written and verbal skills.
- Ability to use a computer proficiently.
