Title: Licensed Vocational Nurse (LVN) – SNF

Location: Quincy, CA 95971

Type: Full-Time | Permanent | Direct Hire

Shift: 3 x 12-hour shifts (Nights)

Compensation: $54,587 – $77,819 annually + Full Benefits

Sign-On Bonus: $5,000

We are seeking a compassionate and skilled LVN/LPN to provide high-quality patient care in a dynamic healthcare environment. This role offers the opportunity to work with diverse patient populations, including individuals with dementia, developmental disabilities, and acute medical needs, while collaborating with a multidisciplinary care team.

Key Responsibilities

• Perform patient assessments, monitor vital signs, and evaluate conditions
• Administer medications, injections, and IV infusions
• Assist with procedures including catheterization, wound care, and respiratory support
• Maintain accurate documentation within EHR systems (Epic, Cerner, Athenahealth)
• Support discharge planning and coordinate care transitions

Qualifications

• Current California LVN license
• Minimum 1 year of LVN experience
• Graduate of an approved LVN program
• Current CPR certification
• Strong clinical judgment, communication, and organizational skills
• Comfortable using EHR systems and computer applications
