Job Summary

We are hiring a Social Services Clinician / Behavioral Health Clinician to join the care team in beautiful Sitka, Alaska.

In this role, you will provide behavioral health assessments, discharge planning, care coordination, and brief therapy for patients from newborn to elderly. You will collaborate with hospital teams and community partners to ensure patients receive the right support and services during transitions of care.

Key Responsibilities

• Conduct behavioral health screenings, assessments, and treatment planning
• Provide brief therapy and continuing care
• Lead discharge planning and patient care conferences
• Coordinate care with hospital teams and community partners
• Support patients with mental health and substance use needs
• Maintain accurate clinical documentation

Qualifications

• Master’s Degree in Social Work, Counseling, Psychology, or related field.
• Licensed or license-eligible LCSW, LPC, or LMFT.
• Experience in behavioral health or discharge planning preferred.
• Ability to obtain Alaska Driver’s License within 6 months
