Rehabilitation Licensed Practical/Vocational Nurse (LPN/LVN)

We’re seeking compassionate and dependable Rehab LPNs/LVNs to provide high‐quality nursing care to patients recovering from injury, illness, or surgery. If you enjoy working as part of an interdisciplinary rehab team and supporting patients through meaningful milestones, this role offers rewarding work with flexible scheduling.

Schedule

• Per Diem
• 12‐Hour Shifts
• 7:00 AM – 7:00 PM (Days)
• 7:00 PM – 7:00 AM (Nights)

What You’ll Do

• Provide direct patient care under the supervision of a Registered Nurse
• Administer medications and treatments per physician and RN orders
• Monitor vital signs, functional status, and overall patient condition
• Assist patients with activities of daily living (ADLs) and mobility in support of therapy goals
• Reinforce patient and family education related to rehabilitation plans and self‐care
• Collaborate with physical, occupational, and speech therapy teams
• Observe and report changes in patient condition promptly to the RN
• Support care planning, safety protocols, and infection control standards
• Document nursing care, observations, and patient progress accurately in the medical record

What We’re Looking For

• Active LPN or LVN license in the state of practice
• Minimum of 1 year of recent rehabilitation, med‐surg, or inpatient hospital experience required
• BLS certification required
• Strong medication administration and clinical observation skills
• Patient‐centered, recovery‐focused approach to care
• Excellent communication and teamwork skills
• Ability to work effectively in a fast‐paced rehabilitation environment
