(SNF) Licensed Practical/Vocational Nurse (LPN/LVN)

We’re seeking dependable and patient‐focused SNF LVNs to deliver high‐quality nursing care in a skilled nursing or long‐term care setting. If you value continuity of care, teamwork, and building meaningful resident relationships, this role offers rewarding work with flexible scheduling.

Schedule

• Per Diem
• 8‐Hour Shifts
• Shift start times:
  • 7:00 AM (Days)
  • 3:00 PM (Evenings)
  • 11:00 PM (Nights)

What You’ll Do

• Provide direct nursing care to residents under the supervision of a Registered Nurse
• Administer medications and treatments per physician and RN orders
• Monitor vital signs, resident condition, and changes in health status
• Perform wound care and treatments within scope of practice
• Assist residents with activities of daily living (ADLs) as needed
• Collaborate with CNAs, RNs, physicians, and interdisciplinary care teams
• Communicate changes in resident condition promptly to the care team
• Support care planning, infection control, and safety protocols
• Document nursing care accurately and timely in the medical record

What We’re Looking For

• Active LVN license in the state of practice
• Minimum of 1 year of recent SNF, long‐term care, or post‐acute care LVN experience required
• BLS certification required
• Strong medication administration and clinical observation skills
• Compassionate, resident‐centered approach to care
• Excellent communication and teamwork skills
• Ability to work independently in a fast‐paced SNF environment
