Job Description
Description: To provide direct resident care, administer daily medications, maintain accurate medication records, and assist in maintaining a positive physical and social environment for the residents of the home.
Minimum Qualifications:
· Must be a Certified Residential Medication Administrator (CRMA).
· If not a CRMA at time of hire; Must have one of the following; PSS, RCS, or CNA, or complete within 90 days of hire.
Note: If you are also a CNA in addition to a CRMA, you are not being employed as a CNA and only as a CRMA.
· First Aid and CPR certification.
· Ability to read written directions, comprehend verbal instructions, and write at a level necessary to accomplish the needs of the position.
· Ability to maintain acceptable attendance standards as explained in the attendance policy.
· Ability to maintain a minimum of ten (10) hours of in-service training annually, which will be provided by Magnolia Assisted Living.
· Ability and willingness to perform the Essential Functions of this position as listed below, with or without reasonable accommodation.
· Must be able to pass a background check
· Must be able to pass a drug test
Physical Requirements
· This is primarily a physical position, which requires the following: visual and auditory acuity within normal limits; fine hand coordination; ability to endure frequent standing, walking and transport of residents; bending and kneeling reaching, grasping, pushing and pulling wheelchairs and other equipment; moving of residents (NOT LIFTING DEAD WEIGHT) with assistance as required, such as moving of residents into bed (roughly 40 pounds), up from the bed (roughly 60 pounds), and assisting from a sitting position to standing (roughly 60 pounds).
· This position also requires communication skills such as; the ability to communicate with residents and personnel both reading and writing, professionally. The candidate should also display the ability to remain calm under stress and during the course of various types of incidents including up to end of life.
Universal Precautions
· The candidate should understand that there is a potential for exposure to blood and body fluids, although all precautions should be taken otherwise.
Essential Functions
· The duties for this position include, but are not limited to:
1. Personal Care
· Provide direct care for residents
· Perform or assist with resident ADLs, following Resident Care Plans and flow sheets as a guide to include:
o Bathing and personal hygiene
o Dressing and undressing
o Oral hygiene

· Perform or assist with resident ADLs, following Resident Care Plans and flow sheets as a guide to include (Continued):
o Nail Care (except diabetics) to include foot care
o Shaving
o Incontinent Care
o Toileting
o Changing of bed linens (to include washing)
o Positioning
o Preparation of residents for meals and snacks/nourishments. Assist residents with eating as necessary.
o Prepare all scheduled meals and snacks including breakfast, mid-morning snack, lunch, afternoon snack, and supper in the absence of dietary staff (if applicable to the home).
o Serve meals in the dining room, ensure residents are notified of meal times, and offered snacks as appropriate.
o Cleanup of kitchen and dining facilities after every meal
o Routine housework i.e. bathrooms, garbage removal and dishes during every shift, in addition to common cleaning needs.
o Transfer resident to or from a bed, chair or commode if necessary
o Perform assigned restorative procedures
§ Assistance with standing and/or ambulation
§ Encouragement and direction in self-care or bowel/bladder programs, as indicated.
§ Provide Range of Motion exercises.
o Transportation of residents to activities of choice as needed. Note: Your personal vehicle is not required.
o Regularly observe the physical and mental condition of residents and report changes to the House Manager and Nurse.
o Assist residents with the consumption of medication as prescribed to include:
§ Proper documentation of all medications administered each shift
§ Appropriate and accurate record keeping of medication prescriptions
§ Able to read and follow any and all medication orders
o Evaluate resident vital signs as required, and note where appropriate
o Answer room calls promptly and courteously.
o Greet residents, visitors, and staff with courtesy and professionalism
o Respect the dignity and confidentiality standards and promote resident rights.
o Follow facility fire safety and infection control practices.
o Promptly report any unsafe conditions or equipment and fill out proper forms.
o Accurately document care provided on ADL flow sheets as well as shift notes.
o Performance of emergency first aid procedures when appropriate, including basic first aid, evaluation of vital signs, CPR, the Heimlich maneuver, etc.
2. Administration and Support
· Assist with new admissions, discharges and transfers, as directed by the House Manager and Administrators.
· Report all incidents and accidents including the completion of appropriate documentation.
· Coordinate personal care services with other departments or disciplines.
· Assist others with lifting and other “team” needs, as required.
3. Staff Development
· Attend and participate in one (1) hour monthly training sessions, in-services training, and staff meetings if scheduled at least ten (10) days in advance
