Job Description
  • Provide direct skilled nursing care including but not limited to:

    • Medication administration and monitoring

    • Tracheostomy and ventilator care

    • Tube feedings (G-tube, J-tube)

    • Wound care

    • Seizure management

    • Catheter care

    • Vital signs monitoring and documentation

  • Follow and implement the individual's nursing care plan as developed by the RN and care team.

  • Maintain accurate and timely clinical documentation in accordance with DBHDS, DMAS (Medicaid), and agency policies.

  • Monitor for changes in the individual’s health status and notify the appropriate provider (RN supervisor, physician, case manager, etc.).

  • Provide health education to individuals, families, and caregivers as appropriate.

  • Collaborate with other members of the interdisciplinary team, including behavioral therapists, direct support professionals (DSPs), and case managers.

  • Comply with all state regulations, Medicaid requirements, and DBHDS provider guidelines.

Qualifications:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the Commonwealth of Virginia (unencumbered).

  • CPR/BLS Certification (American Heart Association preferred)

  • Experience in home health, pediatrics, developmental disabilities, or behavioral health preferred.

  • Knowledge of DBHDS waiver programs, including Community Living (CL), Family and Individual Supports (FIS), and Building Independence (BI), is preferred but not required.

  • Must pass all required background checks, including:

    • Criminal history

    • Nurse Aide Registry

    • OIG (Office of Inspector General) exclusions

    • CPS (Child Protective Services), if applicable

  • Strong clinical skills with attention to detail

  • Excellent communication and interpersonal skills

  • Ability to work independently in a home setting

  • Compassionate, patient, and respectful of individual rights

  • Ability to document care in electronic or paper records clearly and accurately

Work Environment:

  • Services provided in private homes or community settings

  • May require flexible hours including evenings, weekends, and holidays

  • May involve exposure to pets, secondhand smoke, or environmental allergens

Compliance & Supervision:

  • Operates under the supervision of a Registered Nurse (RN) per Virginia Board of Nursing regulations and DBHDS waiver requirements

  • Complies with all DMAS documentation and service authorization guidelines
