Job Description
One You Love Homecare (OYLH) Bridgewater caregivers improve the quality of life for our clients. The caregiver must have excellent communication skills, personal care experience with seniors and a heart for working with seniors. The main function of the caregiver is to provide care for clients in a safe, secure, professional environment under the direction and guidance of the skilled (RN) or assigned Client Care Coordinator at OYLH. Caregivers will follow the client’s personalized plan of care, reporting all changes or concerns directly to the agency.
A Caregiver, based on level of experience and attainment of any required state licensure, will assist with the Activities of Daily Living (ADLs) such as bathing, dressing, grooming, personal hygiene, continence care, bed bound care and assistance with safe mobility of the client per the plan of care. Caregivers assigned to cases that require Homemaker Services will assist in Instrumental Activities of Daily Living (IADLs) consisting of, but not limited to, meal preparation, medication reminders, maintaining a safe and clean environment and the overall supervision of the clients’ physical and mental well-being.
A Caregiver will consistently provide compassionate care and socialization to the OYLH clients.

Primary Responsibilities (based on level of care competency and assignment by RN)
· Provides daily routine care with both ADL and IADL tasks as indicated in the Plan of Care. Assignment to a case will be contingent on level of care competency of an individual as demonstrated in the hiring/screening process.
· Cleans rooms and makes client beds.
· Answers client’s calls.
· Engage the client in conversation and activities to provide a stimulating and enhanced environment that will give the client assurance and socialization.
· Observes Universal Precautions.
· Provides client transportation to doctor appointments, outside activities and shopping.
· Provides client laundry and linen changes.
· Light housekeeping.
- Vacuum
- Dust
- Wipe down kitchen
- Sweep/Mop floors
- Straighten and maintain bathrooms after use
· Provides medication reminders.
· Meal preparation and clean up.
· Report observations that require escalation to appropriate management personnel.
· Clock in and Clock out on time for every assigned shift.
· Document all task activity for every shift in ClearCare.
· Communicate clearly to both client and management personnel.
Knowledge and Skills Requirements
· State certification and license in good standing, if applicable.
· High school diploma or GED.
· Valid driver's license, proof of insurance and reliable transportation.
· Exceptional interpersonal skills.
· Candidates must be authorized to work in the United States.
