Position Summary

We are seeking a skilled and compassionate Registered Nurse (RN)/ Licensed Practical Nurse to provide care in a home healthcare agency. The Field Nurse is responsible for delivering high-quality patient care, conducting assessments, coordinating services, and ensuring compliance with all regulatory standards.

Key Responsibilities

• Perform comprehensive patient assessments, including OASIS documentation - RN only
• Develop, implement, and update individualized care plans - RN only
• Provide skilled nursing care and monitor patient condition
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Educate patients and families on care plans, treatments, and disease management
• Complete timely and accurate clinical documentation
• Supervise and evaluate home health aides and LPNs
• Participate in case conferences and care coordination activities
• Ensure compliance with Medicare, Medicaid, and regulatory requirements

Qalifications

• Active Registered Nurse (RN) license in the applicable state
• Graduate of an accredited school of nursing
• Minimum 1 year of clinical experience (home health preferred)
• Knowledge of Medicare/Medicaid guidelines and home health regulations
• Strong assessment, documentation, and communication skills
• Current CPR certification
• Valid driver’s license and reliable transportation
• Ability to work independently and manage time effectively
• Willing to travel to clients' homes to provide care

Work Environment & Requirements

• Travel to patient homes required
• Ability to perform physical tasks including standing, walking, and assisting patients
• Exposure to clinical and home care environments
