Responsibilities:

  • Nurses assist with medication administration and assessment of client’s health
  • Communicate with physicians and ancillary services
  • Act as shift supervisors when a House Manager isn’t present.
  • Nurses also provide daily support to individuals with disabilities, in the individual’s home and community, with activities like personal hygiene, and meal preparation, behavioral intervention, etc. 

Work Schedule:

  • Open positions for 3rd shift
  • Full time, part time, and flexible shifts available

With a long track record, Transitional Living Centers continues to lovingly provide person-centered care for individuals while they live as independently as possible while receiving whichever supports they may need. A family owned and operated business, TLC started in 1964 when Robert and Wilma Trivanovich, a home-builder and a nurse, decided to put their shared expertise to work.

In 1965, they opened a nursing home in Brunswick, Ohio (now Willowood Care Center) and quickly expanded both the building and the business to include 96 beds for individuals with intellectual disabilities in 1972.

In the early 90’s, TLC built 12 small group homes in Valley City and Brunswick. Today, these homes are staffed 24/7 for our clients from 5 to 80 years old.

In 2009, we opened Riverside Habilitation Center, a day programming facility specially designed to meet the needs of the individuals living here at TLC. Located in Valley City, RHC was designed with our individuals in mind. The 14,000 square foot building has multiple spacious activity rooms each providing active treatment and programming for our individuals. Examples of these rooms include an arts and crafts room, a music room, a game room, and a spa/salon.

We have been in the field of health care for over 50 years and in the forefront of the Intellectual Disability (ID), Developmental Disability (DD) Residential Service System for over 40.

We have a dedicated Social Services department to assist you in any way while you consider a placement decision or during the process of admission. They continue to work with you during your loved one’s time with us.

TLC has been recognized by the American Health Care Association, receiving the Silver Quality Award. Each of our homes meet state Life Safety Codes and are Medicaid certified, and licensed by the Ohio Department of Developmental Disabilities.

Our clients enjoy family style living in 4-8 person handicapped accessible homes with 24-hour direct and nursing care.

Total medical needs are met, including dental, vision, psychiatric, and podiatry services. TLC has great relationships with community physicians, clinics, and hospitals that help us meet each individual’s treatment needs.

Physical, occupational, and speech therapy services are available and registered dietician services are provided.

TLC places emphasis on self determination and independence in all settings; making any individual’s preferred community activities available via accessible vehicles.
