HIRING for ALL PRN POSITIONS: RN-LVN-CNA-PT-PTA-OT-COTA-ST-STA

We are looking for an empathetic and passionate Home Health Nurses, (RN and LVN) Certified Nurse Aides and Therapist (PT/OT/ST and assistant therapist) to assist a growing home healthcare company.

Our patients are home bound and mostly elderly and ill. It is necessary to provide each patient with the time and attention necessary to meet his or her needs.

Notes: The position will require travel within the city of Angleton and its contingent counties. This is a per diem position that has potential to transition into full time. Must work with other healthcare professionals to create treatment plans. Monitor patients' vital signs and wound care. Monitor changes in behavior and progress. Monitors and administers medication. Communicates with a referring physician. Participates in professional development activities and maintains professional affiliations. Skill level required will vary depending on patient needs and will include training on G-tube care, trach care, and vent care as well as ADL's, medical management, and other skills. Seeking skilled nurses with the comfortability of working independently and familiarity of OASIS, for RN.

RN's - Responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care. Will work cohesively with LVN and/or CNA in designated coverage areas.

Qualifications:

• Acute Hospice or Home Care experience is preferred but not required

• Comfortable in a role working alone without any assistance

•Must be willing and able to travel to multiple patient's homes daily and provide quality care and follow up while being efficient

•Current & unremarkable RN/LVN License - CNA cert - Therapy License

•Current BLS/CPR certification

• Working knowledge of Kinnser EMR software

Position pays on a bi-weekly basis as a W-2 employee

ALL RATES ARE PER VISIT RATES AND ARE BASED ON EXPERIENCE:

All these positions have the possibility of becoming Full-Time positions

*RN- $115-$130 /per OASIS - $60-$70 /per visit

*LVN- $50-$55 /per visit

*CNA- $25 /per visit

Contract positions:

PT/OT- $60-$75 /per visit

PTA's / COTA's- $50-$60/ per visit
