- Type: 100% Outpatient
- Call: 1:5 telephone only, and supported by nurse advisory line
- Model: Patient Center Medical Home Model
- Start date: 60-90 Days
- Schedule: Mon - Fri with flexible scheduling
- Job Description: Typical outpatient Primary Care services
- Patient Population: All Ages or Adult Medicine
- Patient Load: 18-22 PPD
- EMR: Epic
- Support: 2 experienced APPs | Full admin staff
- Compensation/Benefits:
