Position Summary


The Advanced Practice Provider Mobile Wound Care is a Physician Assistant responsible for the evaluation, management, and longitudinal care of complex chronic wounds in patients homes. The role focuses primarily on lower-extremity venous stasis ulcers, arterial ulcers (commonly in diabetic patients), and pressure injuries in bed-bound or wheelchair-dependent individuals.

The PA functions as a field-based clinical extension of the supervising Interventional Radiologist, providing detailed wound assessments, determining wound etiology, and coordinating multidisciplinary care with home health agencies under physician-directed treatment plans.

Core Responsibilities


Clinical Evaluation & Wound Assessment


  • Perform in-home evaluations of chronic and complex wounds

  • Obtain targeted histories relevant to wound etiology, vascular disease, diabetes, immobility, and infection risk

  • Conduct comprehensive wound assessments, including:

    • Size, depth, tissue characteristics, exudate, odor, and surrounding skin

    • Signs of infection or ischemia

  • Determine primary wound etiology:

    • Venous stasis

    • Arterial / ischemic

    • Pressure-related

    • Mixed etiologies

Care Coordination & Clinical Communication


  • Communicate findings and recommendations directly with the Interventional Radiologist, who directs overall care

  • Collaborate closely with clinical staff to:

    • Implement physician-directed treatment plans

    • Relay and clarify orders to home health agencies

  • Serve as the clinical liaison between the supervising physician and home health nursing teams

Treatment Planning & Follow-Up


  • Assist in developing and adjusting wound care plans under physician guidance

  • Monitor wound progression and response to therapy

  • Identify patients who may require:

    • Vascular imaging or intervention

    • Escalation of care

    • Advanced wound therapies

  • Participate in weekly or bi-weekly follow-up visits, depending on patient acuity

Documentation & Compliance


  • Complete timely, accurate documentation of:

    • Wound evaluations

    • Clinical impressions

    • Communications with physician and staff

  • Support documentation requirements for:

    • Home health coordination

    • Medical necessity

    • Ongoing treatment authorization

Work Environment


  • Home-based patient visits

  • Patients primarily seen weekly or every other week

  • Home health agencies provide more frequent care per physician orders

  • Structured, physician-led model with strong clinical support

Required Qualifications


  • Licensed Physician Assistant

  • Experience in wound care, vascular disease, geriatrics, or chronic disease management preferred

  • Comfort with independent home visits within a physician-directed framework

  • Strong communication and coordination skills

Preferred Skills


  • Familiarity with venous, arterial, diabetic, and pressure ulcers

  • Ability to recognize when wounds may have vascular etiologies requiring intervention

  • Experience working alongside home health agencies

  • Organized, detail-oriented, and clinically observant
