🧾 Job Overview

We are seeking an experienced RN Case Manager to support care coordination, utilization review, and discharge planning in a fast-paced acute care environment.

This role is ideal for seasoned case managers with recent inpatient hospital experience and strong knowledge of InterQual and Epic systems.

 

📋 Key Responsibilities

• Perform care coordination and discharge planning for hospitalized patients
• Conduct admission reviews using InterQual criteria
• Collaborate with physicians, social services, and interdisciplinary teams
• Assess patient needs and arrange services (DME, home care, post-acute placement)
• Manage insurance authorizations, benefits eligibility, and payer communication
• Develop and implement patient-centered plans of care 

 

🚨 HARD STOP REQUIREMENTS

• Minimum 2+ years acute care inpatient Case Manager experience
• Experience must be current within the last 6 months
• Epic charting experience (required)
• InterQual criteria knowledge (required)
• Previous travel experience required (seasoned traveler preferred) 

 

✅ Requirements

• Active California RN License (or pending accepted)
• Strong experience in:
  • Admission reviews
  • Care coordination
  • Discharge planning
• Experience with HMOs, IPAs, and insurance workflows

• Setting: Acute Care Hospital
• Caseload: ~20–22 patients/day
• Patient Population: Adults & Geriatrics
• Support Staff: Social Services
• Weekend Requirement:
  • At least 2 weekend shifts per 4 weeks
• Floating: Not required
