We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide psychosocial support, discharge planning, and care coordination services in an acute care setting. The ideal candidate will collaborate with interdisciplinary healthcare teams to support patients and families through assessment, counseling, referral management, and transition-of-care planning.
Key Responsibilities- Conduct comprehensive psychosocial assessments for patients and families.
- Provide supportive therapy and emotional counseling services.
- Coordinate discharge planning and referral management.
- Collaborate with physicians, nurses, and multidisciplinary healthcare teams to ensure continuity of care.
- Assist patients with community resources, placement coordination, and follow-up services.
- Maintain accurate and timely documentation in accordance with departmental and regulatory standards.
- Support patients in inpatient medicine, surgery, medical/surgical rehabilitation, and related acute care units.
- Participate in care planning meetings and patient advocacy initiatives.
- Minimum 1 year of LMSW experience required.
- Hospital experience strongly preferred.

- Strong communication, interpersonal, and organizational skills required.
- Master’s Degree in Social Work required.
Active NY LMSW license required
