Primary Care Physician (Family Medicine / Internal Medicine)

Salary: $230,000 – $275,000 (varies depending on years of experience)

About the Opportunity

We’re seeking a compassionate and mission-driven Primary Care Physician to provide comprehensive care in a patient-centered, community-focused setting.

This role is ideal for a physician who values whole-person care, enjoys building long-term patient relationships, and is passionate about supporting individuals with both medical and behavioral health needs.

What You’ll Do

• Conduct comprehensive physical exams and evaluate medical and psychosocial histories
• Diagnose and manage acute and chronic conditions across a diverse patient population
• Prescribe medications and manage ongoing treatment plans
• Order and interpret diagnostic tests, including lab work and screenings
• Identify complex cases and collaborate with physicians or specialists when needed
• Apply evidence-based practices to guide treatment and care planning
• Provide clear, supportive health education to help patients understand diagnoses and treatment options
• Collaborate with interdisciplinary teams to ensure continuity of care
• Support referrals and follow-up care for medical, behavioral health, or social needs
• Adhere to infection control protocols and clinical best practices
• Maintain accurate, timely documentation within EMR systems

Required Qualifications

• MD or DO degree from an accredited institution
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Active or eligible for medical licensure in New York
• Minimum of 2 years of clinical experience preferred
• Background in primary care, community health, or addiction medicine
• Experience working with diverse or underserved populations
• Excellent communication and patient engagement abilities
• Collaborative, team-oriented mindset
